SPORTS PHYSICALS: Charlo’s Logan Health Mobile Unit is offering sports physicals on June 24 and July 28 from 9:00 AM to 3:00 PM at the school.
Appointments will be scheduled at three students per hour—please come prepared with:
- Pages 1–2 of the MHSA sports physical form fully completed
- A brief explanation for any “yes” answers
- The back page signed by a parent/guardian
- A contact number
Cost is $30, payable by cash or check at the time of the appointment. Slots are first come, first served.

Parental consent is required. A parent/guardian should attend the appointment if possible. If not, they must be available by phone at the time of the exam to provide verbal consent, or the student cannot be seen.
